your fundraiser. Items
marked with an asterisk (*) are required fields. A copy of
this form will also be sent to the email address that you indicate
below.
NOTE: If
your school is utilizing our "Home Delivery" you
can either instruct your individual customers to call our toll-free
Customer Service line to indicate their replacement items (they
should have received a card in their order that states this as
well), or if you do opt to fill out the below information please be
aware that the items will still be shipped directly to each customer
(versus to the school).
